Our nurses, cna’s physical, occupational and speech therapists along with other staff will compliment your current caregiver in providing care, comfort, and support while still allowing you to be the decision maker regarding your treatment.
Staff and Services Offered
Hometown Homecare will strive to regain your independence as well as to become as self-sufficient as possible. Our Homecare services start within 24-48 hours of discharge from a facility or hospital. We also provide home safety inspections as well as assess and train on the proper usage of assistive devices or fittings of new equipment. Our dedicated staff will coordinate an individualized plan of care based on your specific needs. Our staff is available 7 days a week.
Homecare Medical Director
Our homecare Medical Director works in conjunction with your attending physician and our Hometown homecare team to manage and support your plan of care.
Your Attending Physician
The plan of care will be overseen by the patient’s current attending physician. The Hometown staff will work with this physician to follow any medication changes, treatments and other related medical information as indicated in the plan of care.
The homecare Clinical Director oversees the coordination of care with the Hometown Homecare Team.
Our Homecare Nurses specialize in caring for patients with illnesses. They are responsible for the skilled nursing care provided to patients wherever they might reside.
Our experienced physical therapists will help with the promotion of mobility, functional ability, strength and balance to enhance quality of life.
Our expert occupational therapists strive to help our patients live as independently as possible. Working with patients to regain independence in Activities of Daily Living (ADL’s) is the primary goal. These activities can include but not limited to; bathing, dressing and meal preparation.
Our Speech Therapists help in restoring communication skills with patients to enhance and improve quality of life.
Home Health Aide
Depending upon a patient’s specific need, a home health aide may be added to your plan of care to assist with personal care while being treated by a nurse, physical therapist and/or speech therapist. Assistance may include but not limited to; bathing, assisting in self cares, assisting patient in and out of bed.
